Clinical Study on Insulin Dependent Diabetes mellitus

Abstract
Purpose
: Insulin dependet diabetes mellitus(IDDM) is a rare disease in children, but it will progress rapidly and falls into a diabetic ketoacidosis or coma. Unfortunately, the characteristics and prevalence rate of children with IDDM are not well known in Korea. With a view to recognizing its importance, we studied clinical course of IDDM patients in Chonbuk province, Korea.
Methods
: From January 1981 to December 1993, 43 children with IDDM who were diagnosed at Presbyterian Medical center, Wonkwang University Hostpital and Chonbuk University Hospital were analyzed retrospectively.
Results
: 1) The highest age incidence was in the age group of 11∼14years, and male to female ratio was 1:1.4, and seasonal distribution showed high incidence in winter. 2) Diabetic ketoacidosis was noted 28% of IDDM patients on admission. 3) Ketoacidosis was noted in 12 patients, and the cardinal symptoms were polydipsia, polyuria, and altered consciousness. 4) Laboratory findings at the ketoacidosis showed hyperglycemia, hemoconcentration, severe metabolic acilosis and hypocalcemia. 5) complications in 13IDDM patients follow-up for more the 2 years were diabetic ketoacikosis(5cases), infection(3cases), neuropathy, cataract, nephropathyu and the dosage of mean daily insulin requirement was 0.72±0.19u/kg.
Conclusion
: Many IDDM patients have suffered from ketoacidosis and chronic complications such as cataract, nephropathy. Therefore we need of the further studies of IDDM on a national scales.
