- Serum 2-microglobulin and Retinol Binding Protein and Urine 2-microglobulin and 1-microglobulin in Early Neonatal Period

- Abstract
- Purpose
: For evaluation of usefulness of the serum .2-microglobulin(B2MG), retinol binding protein(RBP), urine 1-microglobulin(A1MG), and B2MG on neonatal renal function and renal tubular injuries, we detected and compared these values in 20 neonates.
Methods
: They were grouped into premature neonate, gestational age was 30-50 weeks(group Ⅰ), and full term neonate, gestational age was 38-40 weeks(group Ⅱ), The group Ⅱ was subdivided into 5 normal full term neonate(group ⅡN)and 5 as phyxiated neonate(group ⅡA) whose 1 min apgar score was below 4. We detected the values of serum RBP, B2MG, urine A1MG, and B2MG on these neonates. Using these values, the clearence of B2MG and fractional excretion of B2MG were estimated
Results
: 1) The mean birth weight was 2.005gm in the group Ⅰ and the mean 1 min. Apgar score 6.9 in the group Ⅰ and 2.8 in the group ⅡA. 2) The mean values of serum RBP were 0.160.06 mg/L in the group Ⅰ, 1.190.07 mg/L in the group ⅡA,.and 0.150.03 mg/L in the group ⅡN respectively. 3) The mean values of serum B2MG were 2.530.78 mg/L in the group Ⅰ, 2.39 0.91 mg/L in the group ⅡA, and 3.982.5 mg/L in the group EN respectively. 4) The mean values of urine A1MG were 25.3411.06 mg/L in the group Ⅰ, 31.80 21.21 mg/L in the group ⅡA, and 28.5017.80 mg/L in the group ⅡN respectively. 5) The mean values of urine B2MG were 2.410.98 mg/L in the group Ⅰ, 2.810.80 mg/L in the group ⅡA, and 2.491.09 in the group ⅡN respectively. 6) The clearance of B2MG was significantly high in group ⅡA compared with group Ⅰ (p<0.05). 7) The fractional excretion of B2MG (FEB2MG) was high in the group ⅡA compared with the group Ⅰ and the group ⅡN (p<0.05).
Conclusion
: It is concluded that we can uses the value of serum B2MG and RBP in the premature and full nterrm neonate as a normal reference value, and the values of the clearance of B2MG and FEB2MG as a predicting the hypoxic damage of renal proximal tubules in the asphyxiated neonate.
