Clinical and Endoscopic Findings in Children with Duodenal Ulcer due to Helicobacter pylori Infection

Abstract
Purpose
: Recently, a wide application of gastrofiberoscopy in pediatrics has revealed that the incidence of peptic ulcer has increased in the past years, and various results were reported about the relationship between duodenal ulcer and H. pylori. The aim of this retrospective study is to investigate the role of H. pylori in duodenal ulcers in children and to help diagnose and treat duodenal ulcers due to H. pylori infection in children.
Methods
: Endoscopic biopsy specimens which were obtained from two different sites of gastric antrum and duodenal ulcer were stained with Giemsa’s stain, and were graded according to the criteria outlined by Prieto et al. From March 1990 to June 1997, we investigated clinical and endoscopic features of 15 consecutive patients diagnosed as having duodenal ulcer at the pediatric department of Seoul Red Cross Hospital. We divided duodenal ulcer patients into two groups : group Ⅰ were H. pylori positive patients and group Ⅱ were H. pylori negative patients following the Prieto et al criteria.
Results
: There were no differences in age and sex distribution, blood types, number of ulcers, serum gastrin level and complications between both groups(P>0.05). The rate of occurrence for duodenal ulcer in group Ⅰ patients were 4.5%(n=22), 9.0%(n=22) and 25.0%(n=24) in H. pylori score 1, 2, and 3, respectively. Among associated endoscopic findings, nodular gastritis was the most frequently observed disorder(53.3%), especially in group Ⅰ. The recovery rate was higher in group Ⅰ patients(83.3%) than in group Ⅱ patients(66.7%).
Conclusion
: We found that the outcome of duodenal ulcer due to H. pylori infection was good because H. pylori was eradicated. Further studies for how to treat and prevent recurrence of duodenal ulcer should be done.
