A Clinical Study of Periventricular-Intraventricular Hemorrhage in Very Low Birth Weight Infants.

Abstract
34 very low birth weight infants weighing 1,500 gms or less, who were admitted to the neonatal intensive care unit of Seoul National University Children’s Hospital, were routinely studied with serial real-time brain ultrasonography from August 1, 1986 to July 31, 1988. Their medical records and the results of brain ultrasonography were analyzed to evaluate propor- tion, severity, timing, clinical findings, outcome, associated periventricular echodensity and the various risk factors of perivetricular intraventricular hemorrhage (PV-IVH). The results were summarized as follows: 1) The proportion of PV-IVH in the study population was 50.0%. 2) The severity of PV-IVH was grade 1 in 7 cases (41.2%), grade II in 4 cases (23.6%), grade III in 3 cases (17.6%) and grade IV in 3 cases (17.6%). 3) In view of periventricular echodensity, periventricular hemorrhagic infarction was detected in 17.6% of PV-IVH and periventricular leukomalacia was detected in 35.3%. The latter was detected in 38.2% of the study population. 4) Poor motility and bradycardia were statistically significant clinical findings associated with PV-IVH (p<0.05). 5) The proportion of very low birth weight infants who died of PV-IVH was 23.5% and was 0% for grade 1, 25.0% for grade II, 33.3% for grade III and 66.7% for grade IV. 6) Multiple logistic regression analysis for the risk factors of PV-IVH showed that lesser ges- tational age and lower arterial Pa02 were the most significant contributing factors (p<0.05).
