Abstract
In childhood, cancer of the ovary is highly unusual. It is responsible for only 1—2% of cancers found in patients under the age of 17. The frequency with which the various histologic types of ovarian neoplasm occur in childhood differs from that in adults. Epithelial tumors account for 70% to 80% of ovarian neoplasms in adults but only 20% in patients less than 20 years of age. The vast majority of ovarian epithelial tumors in childhood and adolescence are benign, so that a malignant ovarian epithelial neoplasm is extremely rare in the premenachal female. We have experienced a case of a mucinous cystadenocarcinoma in a premenarchal child. She had one-month history of vague abdominal discomfort and increasing abdominal girth. Physical examina- tion revealed a soft, smooth, mobile, fetal head-sized abdominopelvic mass. Abdominal ultrasonogra- phy showed an irregular thin walled huge pelvic mass with mixed echogenicity. At laparotomy, the patient had no ascites. A 23 x 15 x 13 cm smooth, cystic left adnexal mass was removed. The cut surface revealed solid and cystic portions filled with variable amount of chocolate colored fluid. A histologic diagnosis was mucinous cystadenocarcinoma. She was treated with unilateral salpingo- oophorectomy with subsequent survival and apparently normal menarche at 4 months after operation. She has no evidence of disease after 12 months of follow-up with chest P-A, abdominal ultrasonogrpa- hy and CA 125 value. A brief review of the literature was made.
