A Comparative Study of Therapeutic Effect of Intravenous Gammaglobulin plus Aspirin Versus Aspirin Alone in Kawasaki Syndrome.

Abstract
We compared the efficacy of intravenous gamma globulin plus aspirin with that of aspirin alone in reducing the frequency of coronary artery abnormahties and changes of clinical coures in children with acute kawasaki syndrome in the children of 44 cases who were admitted in K.H.U.H from Mar. 1986 to Fab. 1988. Group A: High dose aspirin (60〜90 mg/kg/D) during febrile stage and then switched to Low dose aspirin (5~10 mg/kg/D) during afebrile stage. Group B: Intravenous gamma globulin (400 mg/kg/D) for 5 consecutive days plus aspirin as same as the above method. The results were as follows; 1) The duration of fever after initial treatment was significantly shorter in Group B (1.8±0.9 days) tham Group A (3.5±3.2 days) (p<0.01). 2) Changes in leukocyte, hemoglobin and platelets were not different in both group. 3) After 28 days treatment, the value of ESR was more rapidly decreased in Group B (13.3±11.5 mm/hr) than Group A (28.5 ±23.0 mm/hr) (p<0.01). 4) At admission, the incidence of the abnormal ECG findings was 50% in each groups and after 2 weeks of treatment, Group B (abnormal: 14%) was more rapidly normalized than Group A (abnormal: 38%) but it was not significant (chi = 1.03, p>2.0). 5) In 2D-Echocardiography, coronary aneurysms were more frequent in Group A (27.3%) than in Group B (18.2%) but it was not significant (chi=5.2, p>0.05).
