Analysis of Asymptomatic Proteinuria in School Children.

Abstract
Uriary mass screening program for the detection of proteinuria had been performed for a school- age population in the city of Seoul since 1981. Annually, about half million students, who comprised 3 different age group, corresponding to the averages of 11,14, and 17 years, had been examined. A systematic clinical evaluation was designed for students with proteinuria identified in the mass screening through ’87 and *88 school years to determine the prevalence of asymptomatic proteinuria, and to estimate the risk of underlying renal diseases. The screening program was carried out in 3 steps. Of 557,048 and 563,951 students who participated in the mass screening in ’87 and ’88, respectively, 2,024 (0.36%) and 1,908 (0.33%) were noted to have proteinuria, respectively. There was no difference in the prevalence of asymptomatic proteinuria by sex, and it peaked at the age of 14 years. Total of 415 students who complied the 3rd test at the hospital showed the following pattern of proteinuria; Orthostatic Proteinuria 54.2%, Transient Proteinuria 17.9%, Proteinuria and Hematuria 18.5%, Constant Proteinuria 8.0%, Transient Proteinuria and Hematuria 1.0%. Renal pathologic studies on 27 students with proteinuria and hematuria or constant proteinuria revealed 15 IgA Nephropathy, 3 Membranoproliferative Glomerulonephritis, 2 Membranous Glomerulonephritis, 1 Focal Segmental Glomerulosclerosis, 1 Reflux Nephropathy, 1 Diffuse mesangial Proliferative Glomerulonephritis and 4 mild Focal Glomerulonephritis. Among them, 3 (2 MPGN, 1 MGN) were related with Hepatitis B virus infection.
