Changes of Plasma Atrial Natriuretic Peptide and Antidiuretic Hormone in Congenital Heart Disease.

Abstract
Human atrial myocytes secrete atrial natriuretic peptides (ANP) into the plasma, which causes rapid and marked natriuresis, diuresis and vasorelaxation. The releases of renin, aldosterone, and antidiuretic hormone are inhibited by ANP in vivo and in vitro experiments. Various atrial natriur- etic peptides were isolated and synthesized. Plasma ANP concentrations usually elevated in conges- tive heart failure, chronic renal failure, pulmonary arterial hypertension, tachyarrhythmia, and nephrotic syndrome. However, there are a few reports about the plasma ANP concentration in child with congenital heart disease. Furthermore, The relationship between the plasma ANP and hemodynamic status was not investigated, sufficiently. So we measured the plasma ANP, ADH, and osmolality in peripheral vein and right atrium in congenital heart disease during cardioangiography using nonionic contrast media, the results were as follows: 1) Plasma ir-ANP concentration was significantly elevated in congenital heart dissase than control group (P<0.05). The plasma ir-ANP levels in patient with congenital heart disease were correlated with Qp:Qs (P < 0.05). 2) No significant difference in ir-ANP levels was observed between right atrium and peripheral vein, but higher levels of ir-ANP were observed in right atrium. 3) Plasma ADH levels in congenital heart disease group were higher than in control group (P<0. 05). 4) After cardioangiography, the plasma ir-ANP levels were decreased but plasma ADH,osmolality levels were increased, significantly.
