- Abstract
- Clinical studies on the 82 patients of sympomatic urinary tract infection who were admitted to
pediatric department of Wallace Memorial Baptist Hospital from August 1984 to July 1987 were
subjected in this study.
The result were as follows.
1) The incidence was 0.79% of total pediatric inpatients during the same period.
2) Age incidence disclosed under 1 year in 49 patients(59.7%) and under 6 year in 76 patients(92.6
%)
3) Fifty.four patients were male and 28 patients were female, with male to female ratio of 1.9:1.
Under one year of age, male(39 patients) was more commonly affected than female(10 patients) with
the ratio 3.9:1
4) Among clinical findings of total patients, fever(76.8%), irritability, poor feeding, vomiting,
diarrhea, dysuria, frequency and gross hematuria were noted in the order of decreasing frequency. In
the younger age groups, the symptoms were more likely to be nonspectfic and systemic.
Fever was most common in the age group of 1-11 month(89.5%) than any other age groups.
5) Hematologic findings showed leukocyosis( > 15,000/mm³) in 34 patients(41.5%), low hemoglobin
(≤10 gm/ dl) in 17 patients(20.7%), evevated ESR( > 20 mm/hr) in 57 patients(69.5%).
The C-reactive protein was positive in 50 out of 58 patients(86.2%) and in the age group of 1-11
month, the C-reactive protein was most commonly positive(96.8%).
6) Urinalysis disclosed pyuria in 74 patients(90.2%), proteinuria in 32 patients(39.0%) and hematuria
in 16 patients(19.5%).
7) E. coli was the most common strain(67 ratients, 81.7%) and Klebsiella, Proteus, Enterococcus
were common in the desending order.
8) In vitro antimicrobial susceptibility test showed E. coli was sensitive to moxalactam(100%),
amikacin(96.9%), obramycin(94.2%) and gentamicin(89.6%)
9) Intravenous pyelography was performed in 29 patients, and 16 patients(55.2%) of them showed
abnormal findings. 9 patients had hydronephrosis and/ or hydroureter
10) Voiding cystourethrography findings in 18 patients showed vesicoureteral reflux in 9
patients(50.0%). Associated anomaly was in 3 patients.
11) Out of 63 febrile patients, 41(65.1%) patients became afebrile within 48 hours after onset of
treatment.
Urine became sterile in 58 out of 82 patients, 48 hours after onset of treatmenr.
