Clinical Observation on Admitted Patient in Nursery.

Abstract
Clinical observation was performed on 653 cases of neonates who were admitted in nursery, N ational Medical Center from January 1984 to December 1985. The results were as following; 1) Five hundred and seventy.eight newborns were admitted to nursery among -2102 Iive birth delivered at National Medical Center during the study period. The admission rate was 27.5%. 2) By the delivery modes, the heighest admission rate was seen in vaccum extraction group (45.8 %), followed by C-Section (28.8%) and spontaneous vaginal delivery (25%). 3) Birth weight distribution was followed. The percentage distribution of all admission cases for birth weight was 34.9%. For 3001-3500 gm, 22.1 % for 2500-3000 , 17% for 3500-4000 gm or less. 4) The early mortality rate was 18.0% per 1000 live birth during 2 years (1984-1985). 5) Of all neonatal deaths in nursery, 49 died among the 653 admission cases of 2 years (7.5%). 6) Among the neonatal deaths. 74% died within 1 week after birth. Among these cases, 15% died within 24 hours after birth, 19% and 40% died within 1-2 days and 3-7 days respectively. 7) Within 24 hr after birth, neonatal death was 15% of all death cases. Between 1st day and second day (19%), between 3rd & 7 days (40%), 7 days after birth, neonatal death was 26% of all death cases. 8) Disease distribution of the admission cases were neonatal hyperbilirubinemia (30.8%), meconium aspiration pneumonia (16.0%), prematurity (9.0%), urinary tract infection (5.2%), birth trauma (3.5%) in the order of frequency. 9) The diseases of neonates who needed ventilator at nursery were respiratory distress syndrome, meconium aspiration syndrome, sepsis, biπh trauma and meningitis. In the respiratory distress syndrome group, the average gestational age was 32 weeks, birth weight, 1708 gm and its mortality was 66%. In the meconium aspiration group, 42 weeks, 2817 n and 33.3% respectively. 10) High risk factors were C.Section (18.4%), birth weight under 2500 gm or over 4000 gm (15.3%) and meconium stained (15.0%) in the order of frequency. 11) The most common microorganisms from the admitted infants in nursery were E. coli, Staph. aureus, Klebsiella. and Enterobacter in the order of frequency.
