Clinical Evaluation of Bleeding Tendency in Childhood.

Abstract
Four hundred and ninety-nine cases of bleeding disorders in childhood who were admitted to department of pediatrics, Soon Chun Hyang University Hospital during 10 years from July 1977 to June 1987 were clinically evaluated. The results obtained were as follows: 1) The incidence of bleeding disorders was 2.97% of total admitted patient. There was no signifi- cant difference in annual incidence. 2) The peak age of incidence was 4-6 years (23.9%) and the sex ratio of male (61.5%) to female (38. 5%) was 1.6:1. 3) Anaphylactoid purpura (29.9%) was the most common disease which was followed by D.I.C. (25. 9%), leukemia (11.6%), I.T.P. (8.2%), hemorrhagic disease of newborn and infant (7.6%) and aplastic anemia (4.6%). 4) Anaphylactoid purpura, leukemia, aplastic anemia and non-Hodgkin lymphoma were most common in 4-6 years and I.T.P. was common in lMo-3 years of age and 4-9 years and D.I.C. was common under the 3 years of age. 5) The major clinical manifestations were purpura (100%), melena (26%), epistaxis (5%) in vascular disorder and purpura (56%), epistaxis (43%), easy bruise (32%) in platelet disorders and oozing from injection site (75%), purpura (46%), hematoma (35%) in plasmatic disorders. 6) The platelet count was below 50,000/cu mm in 72.4% of leukemia, 97.6% of I.T.P. and 91.3% of aplastic anemia. In hemorrhagic disease of newborn and infant, PT was prolonged in 96.9% of the cases and all cases of the hemophilia A showed prolonged CT and PTT. 7) The overall mortality rate was 18.0% and peak age in mortality cases were under the IMo old, the most common cause of death was D.I.C., in which 61 among 129 cases (47.3%) were dead.
