Abstract
This study was undertaken to eluidate lipoprotein patterns in patients with childhood nephrotic syndrome by response to initial steroid therapy, serum albumin level in MCNS and other histologic classifications. The result were summerized as follows. 1) There were 19 cases (52.8%) of abnormal lipoprotein patterns by Fredrickson classifications, type Ila were 6 cases (16.7%), type lib were 1 cases (2.8%) and type IV were 12 cases (33.3%) respectively. 2) In non-biopsy group, but suspected to MCNS by clinical finding, laboratory data and response to steroid therapy, abnormal lipoprotein patterns were 6 of 12 cases (50.9%), type Ila were 2 cases (11.7 %) and type IV were 4 cases (33.3%) respectively. Biospy was done and confirmed to MCNS, abnormal lipoprotein patterns were 10 of 17 cases (50.8 %), type Ila were 2 cases (11.7%), type lib were 1 case (5.9%) and type IV were 7 cases (41.2%) respectively. 3) The lipoprotein patterns in response to inital steroid therapy in poor response group; normal patterns were 2 cases (40.0%), type Ila were 2 cases (40.0%) and type IV were 1 case (20.0%) In partial remission group; normal patterns were 7 cases (38.9%), type Ila were 2 cases (11.1%), type lib were 1 case (5.5%) and type IV were 8 cases (44.4%). In complete remision group; normal patterns were 5 cases (83.3%) and type IV were 1 case (16.7%) respectively. 4) The composition percentage of lipoprotein in response to initial steroid therapy, in poor response group; HDL was 29.89±8.65(%), VLDL was 15.0±8.40(%) and LDL was 55.29± 6.91(%) respectively. In complete remission group; HDL was 27.7±7.11(%), VLDL was 18.28±6.09(%) and LDL was 54.00±3. 92(%), respectively. 5) The relationship between serum albumin level and lipoprotein patterns in childhood nephrotic syndrome, increase percentage of normal lipoprotein patterns and decrease type IV proportion in high serum albumin level.
