- A Comparative clinical study of Intussusception with Barium or Surgical Reduction.

- Abstract
- A chinical study was performed on 294 cases of intussusception in infants and children, who were
admitted to the Department of Pediatrics, Yong Dong Severance Hospital from January, 1983 to
December, 1986. 240 cases were treated by barium reduction, were analysed clinically and were
compared with 54 cases treated surgrally. The results were as followed:
1) As for the peak incidence of intussusception, 30.4% of the patients with intussuception treated
by barium reduction and 53.9% treated surgically were younger than 6 months of age. Patients 4
to 36 months of age accouted for 88.4% of the total patients.
2) The percentage of patients admitted within 24 hours of the onset of symptoms was 85.8% in
the barium reduction group ; and 14.8% of the total patients.
3) Symptoms and signs which were more frequently observed in the surgically treated group than
in the barium reduction group included abdominal mass in the epigastrium, currant—jelly stool and
abdominal distension.
4) Simple abdominal X—ray findings which were more predominant in the surgical group than
in the barium reduction group included gaseous distension and fluid levels.
5) The most common type of intussusception was the ileocolic type which was 90.4% in the surgical
group and 44.4% in the barium reduction group. But the ileoileal type including the ileoileocolic
and ileoileocecal type, was more frequent in the surgical group (42.6%) than in the barium reduction
(2.1%).
6) Of the 294 patients with intussusception, barium reduction was performed successfully in 240
cases (81.6%). The recurrence rate of intussusception was 15.0% in the barium reduction group
and 7.4% in the surgical group.
7) Of the 240 patients who had been reexamined by a barium enema, 13(5.4%) had causes of
intussusception and 9 of them (69.2%) were ascribed to lymphoid hyperplasia of the distal ileum.
8) The most common pathologic findings in the surgical group was reactive hyperplasia of the
lymphoid tissue which was observed in 44 of the 54 cases (81.5%).
