Clinical Observation on Hemodialysis in Children with Chronic Renal Failure.

Abstract
There had been total 21 children with end stage renal disease in whom hemodialysis was done in the pediatric dialysis unit of Seoul National University Children’s Hospital during the period from 1986. 3. 31 to 1987. 11. 30. And we took a retrospective review of their clinical data, and the results were as follows 1) Primary glomerular disease, H enoch-Schdnlein nephritis, and reflux nephropathy were the leading causes of chronic renal failure in these patients. 2) Maintenance hemodialysis was done in a frequency of 1 hemodialysis per 2.7 patient day, and average 1 pint of packed RBC transfusion was required per 21.4 patient day during follow up period. 3) Technical problems including occlusion and/or infection of vascular access were more common than medical complications among total serious complications associated with hemodialysis, but the overall incidence of serious complications was minimal. 4) Nearly all of the 8 patients who were followed for more than 1 year had a fall-off in growth velocity during hemodialysis. 5) On the last follow up, 4 cases expired, 3 showed successful post-renal transplantation state, 2 had maintained continuous ambulatory peritoneal dialysis and 10 were still on maintenance hemodialysis in our dialysis unit. In conclusion, hemodialysis is one of the acceptable long-term therapeutic modality for children with end stage renal disease. However it is essential to establish adequate support in every aspect for the better overall quality of life of the patient.
