In certain state, air can leak from the ruptured alveoli into the pulmonary interstitium and movement of this air along the vascular structures may result in pneumomediastinum. We experienced a case of spontaneous pneumomediastinum, in a 3 day-old male infant (43+5 wks gestational age) who was delivered in meconium stained state.
