Abstract
C-peptide can be used as a measure of endogenous insulin secretion in insulin treated diabetes mellitus. Pancreatic p cell secretory activity was measured in 11 patients with insulin dependent and 2 patients with non insulin-dependent diabetes mellitus by means of serum C-peptide response to 1 mg of glucagon subcutaneously. Serum C-peptide was increased to near normal range in the remission phase and decreased progressively with duration of diabetes mellitus. During the first 5 years after onset residual beta-cell function was demonstrable in majority of insulin treated patients. Stable patients had a higher C-peptide concentration than unstable ones. There was a positive relationship between C-peptides and age at onset but negative corrlation to duration of diabetes. There were also negative relationship between C-peptides and. fasting blood glucoses and negative correlation to HBAiC. These results suggest that early detection and intensive treatment of diabetes before severe metabolic disturbance may help preserve pancreas beta-cell function.
