- Abstract
- A clinical study was done on 39 patients, 21 with omphalocele and 18 with gastroschisis,
born at II Sin Women’s Hospital between Jan. 1976 and Dec. 1982. The following results
were obtained.
1) In omphalocele 57.1% of the affected babies were males this being slightly higher than
the 5.8% in overall distribution of total deliveries but in gastroschisis a reverse of the normal distribution was seen and 55.5% of the affected babies were female.
2) 28.6% of the babies with omphalocele and 44.5% of those with gastroschisis were
premature both, of which were much higher than the overall incidence of 8.6% among total deliveries.
3) The method of delivery, in order of frequency was spontaneous vaginal delivery、cesarean section, assisted breech delivery and vacuum extraction.
4) In cases of omphalocele the distribution of birth weight showed a. fairly symmetrical
curve but in cases of gastroschisis 72.2% of the babies were less than 2,500 gm.
5) 50% of babies with omphalocele and 23.1% of babies with gastroschisis had severe birth hypoxia.
6) 11(52.4%) of babies with omphalocele and 3(16.7%) of those with gastroschisis had associated anomalies, and the major anomalies in order of frequency were gastrointestinal,urogenital, and cardiac anomalies.
7) Associated maternal disease & labour complications were found in 60% of cases of omphalocele and gastroschisis. In the observed 30 cases the complications in order of frequency were maternal anemia, prolonged rupture of membrane, hydramnios, fetal distress and puerperal pyrexia. Of these the most significant was hydramnios, and incidence of 13. 3% compared to 0,57% in the total obstetric population.
8) 15(38.4%) of babies with omphalocele or gastroschisis had other complications. In the
observed 23 cases the complications in order of frequency were septicemia, respiratory distress syndrome, gastroenteritis, anemia, jaundice, pneumonia.
9) 7 cases (46.7%) of omphalocele and 9 cases (53%) of gastroschisis were treated with operative procedures. There was a postoperative mortality rate of 14.3% (1 case) with omphalocele, but 55.5%(5cases) with gastroschisis.
10) There was an overall mortality rate of 61.9% (13 cases) with omphalocele, 55.5%-(10 cases) with gastroschisis. The major causes of death were associated anomalies, stillbirth,birth hypoxia, and infection.
