Clinical Observation of Tricuspid Atresia.

Abstract
Twenty one cases of triciispid atresia which were diagnosed at the department of Pediatrics, Seoul National University Hospital during past twelve years and eleven months were reviewed and analysed. The results were as follows. 1) There were 8 males and 13 females. According to anatomical classification, 15 cases were in type 1, 4 cases in type 표 and 2 cases in type M. According to radiological classification, 16 cases were in group A, 5 cases in group B and none in group C. 2) Heart disease was noticed at birth in 13 cases, 7 cases within 2 months and all within 6 months of age. 3) Cyanosis was detected in 95% of all, heart murmur in 86%, dyspnea in 81%, cardiomegaly in 71%, clubbing in 67%, anoxic spell in 48%, hepatomegaly in 38% and heart failure in 24%. 4) Electrocardiogram showed left axis deviation in 17 cases(81 %), normal axis deviation in cases(14%) and right axis deviation in 1 case(5%). Atrial enlargement was observed in 13 cases including 11 cases of right atrial enlargement and 2 cases of left atrial enlargement. 5) Systemic arterial oxygen saturation was distributed in wide range with the mean value of 69.5%. Mean value of group A was 68% and 74.3% of group B. 6) In 13 cases right atrial “a” wave elevated above 10 mmHg and in 7 cases pressure gradient between right atrium and left atrium was above 2 mmHg. Balloon atrial septostomy was performed in 2 cases. 7) Hemoglobin level was distributed from8.9 gm% to 24.4 gm% and the mean value excluding the case of 8.9 gm% was 18. 5 gm%. 8) Three cases were associated with left superior vena cava, 2 cases of single ventricle,single atrium and anomalous pulmonary venous return, 1 case of dextrocardia with situs, inversus and patent ductus arteriosus were seen. 9) Surgical intervention was performed in cases with the surgical mortality of 54. 5%. Three cases operated with Kreutzer procedure, 1 case with Blalock-Taussig shunt, and 1 case with Glenn anastomosis followed by Fontan operation were alive.
