Abstract
W« studied the guidelines on the serum bilirubin concentration of 15 mg% for the management of neonatal jaundice and compared the group of serum bilirubin concentration below 15 mg% ■and the group of serum bilirubin concentration above 15 mg%. 83 cased of icteric infants without any risk factors for kernicterus, ahove 12 m.g% in serum bilirubin concentration, were born at the St. Benedict hospital from August 1979 to July 1980, were studied. The results were as following: 1. Among the 83 cases, 43 cases(51.8%), the group of serum bilirubin concentration below 15 mg%, had spontaneous remission without phototherapy and 40 cases (48.2%), the group of serum bilirubin concentration above 15 mg%, received phototherpy during the period from 72 hours to 96 hours. 2. Between the group of serum bilirubin concentration below 15 mg% and the group of serum bilirubin concentration above 15 mg%, the results showed no significant difference in the month of birth, the gestational age, the weights of newborn infant, the Apgar score, and the delivery method. 3. The incidence of neonatal jaundice according to delivery method was significantly higher in infants born by cesarean section (9.7%) than in both infants born by spontaneous vaginal deliveries (3.2%) and vaccum deliveries(4.5%). 4. The mean serum bilirubin concentration on 3rd day of life was 12.37 mg% in the group of serum bilirubin concentration below 15 mg%, was 13.27 mg% in the group of serum bilirubin concentration above 15 mg%. 5. Among 40 cases (the group of serum bilirubin concentration above 15 mg%), 4 cases was observed until 18 mg% without special treatment, but 1 case was done exchange transfusion because its serum bilirubin concentration had been above 20 mg%. We would like to suggest that the newborn infants without any risk factors of kernicterus need not special treatments until serum bilirubin concentration become to be 15 mg% after the 3rd day of life. .
