Clinical Studies of Streptomycin Intrathecal Infusion in Tb Meningitis

Abstract
A total 45 cases of Tb menigitis who were admitted at the Hanyang University Hospital during the past two and a half years from May 1972. to Dec. 1974,were divided into two groups as one group(19 cases) which was only treated with standard therapy (1NH+PAS+S-M) and the other group (26 cases) which was treated with standard therapy combined with streptomycin intrathecal infusion therapy. We compared the results of both two groups after 8wks treatment and also analysed clinically. Following could be summerized: 1. The common chief complaints in order of frequency were vomiting (36%), fever (22%), convulsion (11%), irritability (7%) and unconsciousness (J%'). 2. The pictures of chest X-ray of tuberculous meningitis children showed normal (40%), primary Tb (38%) and miliary Tb (22%). 3.Between morbidity and age distribution of Tb meningitis, followings were observed that: under 3 years age group showed the highest morbidity (64%) and over 3 years agegroup presented 20%. 4.The mortality rate was 31% in under 3 years age group compared to over 3 years age group (10%). We could see the significant diffenence in different age group. 5.The relation between the duration of illness and level of consciousness was that; the most common within lwk 3% among the conscious group and between 2 to 3wk 45% among the unconscious group. 6.The findings of CSF were that; most cell counts were 201~300/mm3 (27%), most sugar level 31~40mg% (38%) and the most common protein level 101~ 150 mg% (29%). 7.The difference of sugar level in CSF and prognosis was that; the motality rate was 35.2% at below the 20mg% sugar level group and 3. G% in above the 20 mg% sugar level group. We noticed the significant different motality according to CSF sugar level. 8. The results after 8wks treatment were that; 91% survival rate was showesd in conscions group among them 10% was bad result, 72% survival rate in unconscious group among them 33% was bad result 9.The compareson between the result of A group (standard treatment) and B group (standard treatment+streptomycin intrathecal infusion) was that 68% survival rate in group “A,” among survivals, bad result was 15%, 92% survival rate was shown, in group “B, among survivals bad result was 25%.
