Abstract |
Bone marrow cell cultures (GM-CFU) were performed in the 15 cases of children with aplastic anemia and the correlation with the clinical course were investigated. They have been treated with androgen with or without low dose prednisone and supportive care. Bone marrow cell cultures from 8 children without any evidence of aplastic anemia were performed as a control. 1)In control group, colonies were 26 to 57, mean 40.50±13.58.
2)In the patients of aplastic anemia, colonies were 0 to 28, mean 5.74±9.06 (p<0.001). 3) In 15 cases of aplastic anemia, 2 were died within 6 months after the diagnosis. Their bone marrow cell cultures revealed 0 colony and 0 cluster. Their Lynch indeces at the time of diagnosis were high, above 0.041. The other 13 patients are still alive(6 months to 121 months). Four of them are in complete remission, and their Lynch indeces were much lower. The percentage of nonmyeloid cells in bone marrow was inversely correlated with the clinical course.
4)Initial grading of severity according to the peripheral blood findings was not well correlated with the clinical course (sensitivity 50%). The patients who revealed one or more colonies on their bone marrow cell culture showed favorable clinical course (sensitivity 88%). In predicting the clinical course of the aplastic anemia,the result of the bone marrow cell culture would be useful in addition to the findings of the peripheral blood and bone marrow cellularity.
