Abstract
Retrospective clinical analysis of 5 and a half yeas experience with artificial ventilaion on 187 neonas in NICU of National Medical Center, from Jan. 1985 to Dec. 1990, was done. The results were as follows; 1) Number of patients, who required respiratory therapy with aritificial ventilation, was 187 and the percentage of these patients to all hospitalized neonates was 11.4%. 2) Number of male patients was 107(57.2%) and of femal was 80 (42.8%), and male to female ratio was 1.3:1. 3) Major neonatal conditions requird the artifical ventilation were prematurity incluidng apnea of prematurity (70.1%), hyaline membrane disease 59.9%, meconium aspiration syndrome 13.4%, perinatal asphyxia 10.2%, pneumonial 4.3% and others. 4) Major high risk factors were prematurity, institutional baby, acidosis, cesarean section delivery, very-low-birth-weight infant, low Apgar score and meconium-staind infant in the order of frequency. 5) Complications were air-leak syndrome including pneumothoras 12.3%, sepsis 8.6%, intracranial hemorrhage 8.6%, intracranial hemorrhage 8.6%, gastrointestinal or pulmonary hemorrhage 4.8%, bronchopulmonary dysplasia 1.6% and visual field defect ith some evidnce of retinopathy of prematurity 1.1%/ 6) The incidence of case, which needed intensive respiratory therapy including endotracheal intubation within 24 hours of age, was esarean sectin delivery 78.8%, prematurity 44.6% and full-term infant 50%. The case of cesarean delivery needed artificial ventilation earlier than the case of vaginal delivery. 7) Overall mortality was38.9%. The mortality of the inant of gestational age below 3 weeks was 58.5%, that of gestational age between 37 week and 42 weeks was 37.5% and that of postmaturity was 55.6%. 8) The bateria cultured at the tip of endotracheal tube was gram-negative organism in 66 cases and gram-positive organism in 28 cases. 9) Among expired cases, early mortality rate within 7 days was 61.8%. Eighty ive point three percent died within 4 weeks and only 14.7% died after 4 weeks.
