Abstract |
The systemic lupus erythematosus (SLE) is a chronic inflammatory disease involving skin, kidney,
serous membrane, nervous system and other organs.
We experienced a case of SLE accompanied with nephritis and arteritis on the region of the left
thigh, diagnosed by clinical, serologic, immunologic, radiologic and pathologic evaluations.
A 15 years old female patient was admitted to pediatric ward due to sustained pain on left thigh.
She exhibited symmetric erythematous rashes on the face, intermittent moderate fever, enlarged
cervical lymphnodes and hepatosplenomegaly on admission. The hematologic findings revealed
anemia on peripheral blood and erythroid hyperplasia on bone marrow. The urine findings revealed
proteinuira, hematuria and granular casts. The results of ANA, LE, VDRL and Commbs’ test were
positive serologically and the level of C3 was 20 mg/dl. The femoral angiography showed decreased
vascularity, beading of the vascular wall and narrowed vascular lumen on left side. There were
massive granular deposits of the immune complex, complements and electron dense materials on
renal bipsy specimen compatible with diffuse proliferative lupus glomerulonephritis (WHO Class IV).
