Abstract |
Clinical analysis of 48 neonates who treated with Mechanical ventilation in NICU at Won-Kwang
University Hospital from February 1987 to July 1990 was performed.
The results were as follows;
1) According to diagnostic categories, hyaline membrane disease was the most common in 26 cases
(54.2%), and then postasphyxia syndrome 10cases (20.8%), apnea of prematurity 3cases (6.2%),
sepsis 3 cases (6.2%),in their order.
2) 29 cases (60.4%) were bom at WUH and others were bom elesewhere, and male to female ratio
was about 1.4:1.
3) Premature infants below 37 wks of gestational age were 32 cases (66.7%), and hyaline membrane
disease were premature in 25 cases (96.2%).
4) Low birth weight infants below 2,500 gm were 36 cases(75%) and very low virth weight infants
below 1,500gm were 14cases (29.2%). In 26 cases of hyaline membrane disease, infants below 2,500
gm were alll and below 1,500gm were 11 cases (42.3%).
5) According to selected mode of mechanical ventilation, CPAP mode was used in 7 cases (14.6%)
and IPPB in 41 cases (85.4%). Assist ventilation was initiated before 24 hrs of age in 44 cases (91.7%)
and in total duration of mechanical ventilation, 36 cases (75.0%) were below 3 days and 5 cases (10.
4%) above 7 days.
6) Complications were seen during mechnaical ventilation were infection (20.8%) including sepsis
and pneumonia, extrapulmonary air leaks (14.6%), intracranial hemorrhage (12.5%), in their order.
7) Total survival rate was 35.4%. For each diagnostic categories, the survival rate in hyaline
membrane disease was 30.8%, postasphyxia syndrome 30% and apnea of prematurity 100%.
The survival rate in infants less than 999 gm was 0%, 1,000〜 1,499 gm 60%, 1,500—1,999 gm 41.7%
and more than 2,500 gm 27.3%, and in infants of gestational age below 28 wks was 12.5%, 29〜3 wks
50%, 33-36 wks 20%.
8) Follow up observations were performed in 17 survived infants for the period of 3 months to 3
years and long term sequalaes were observed in 5cases (29.4%) as spastic quadriplegia, seizure
disorder, bronchopulmonary dysplasia, delayed development and retinopathy of prematurity.
