Abstract |
In order to estimate the influence of systemic hemodynamic change in cerebral blood flow before
and after closure of ductus arteriosus in premature mewborn infants, blood flow velocities of internal
carotid artery (ICA), anterior cerebral artery (ACA) and middle cerebral artery (MCA) were measured
in 17 premature newborn infants by pulsed Doppler ultrasonography. The data was compared with
those of normal full-term infants by Pulsatility Index (PI) which calculated from measured systolic
and diastolic blood flow velocity. The results obtained were as follows:
1) Systolic and diastolic blood flow velocities measured after ductal closure were increased more
significantly in ICA, ACA, MCA than those measured before ductal closure (p< 0.001).
2) Pi’s calculated before and after ductal closure were 0.79±0.08, 0.72±0.07 in ICA, 0.74±0.06, 0.
68 ±0.06 in ACA, 0.73 ±0.06, 0.67±0.06 in MCA, which result were significantly different before and
after ductal closure (p< 0.001).
3) Mean systolic blood pressure was 72 mmHg, which was not changed with ductal closure, but
mean diastolic blood pressure was increased significantly from 32 mmHg to 36 mmHg with ductal
closure (p<0.05).
4) There were no correlations bewteen gestational age, initial body weight, cerebral blood flow
velocity nor PI.
5) Pi’s of premature infants calculated before ductal closure were less than those of full-term
newborns, but there was no difference between two groups after ductal closure. |
