| Abstract |
|
A clinical assessment was carried out in 37 cases with Extrahepatic biliary atresia (EHBA) and 35 cases with Neonatal hepatitis (NH), who were admitted to the Dept, of Pediatrics and Pediatric Surgery, Seoul National University Hospital, during the 30 months from January 1980 to June 1982. The following results were obtained;
1)By sex distribution, 40% of EHBA and 69% of NH were boys. 2)By birth weight distribution, 3% of EHBA, and 37% of NH were under 2.5 kg, and by order of birth, 68% of EHBA and 40% of NH were first born. 3)The incidence of jaundice within 2 weeks of life was 70% in EHBA and 37% in NH, and the incidence of acholic stool within 2 weeks of life was 57% in EHBA and 14% in NH. Hepatomegaly was noted in 78% of EHBA and 51% of NH and spleen was palpated in 40% of EHBA and 49% of NH. 4)The statistical differences of total and direct serum bilirubin level between EHBA and NH were not significant but those of SGOT, SGPT and G-GTP level were significant. 5)By 131l-Rose Bengal fecal excretion test, below 10% of intravenous injected doses were detected in 100% of EHBA and 20% of NH, and by 99mTc-HIDAscan, no radioactivity of G-I tract was noted in 100% of EHBA and 65% of NH. 6)No bilirubin pigment in duodenal fluid was noted in 100% of EHBA and 7% of NH. 88% of EHBA and 19% of NH were above 2 of G-GTP/SGOS ratio. 7) In 25 cases of EHBA which Kasai’s operation were done, postoperative bile flow was detected in 19 cases and clinical improvement was noted in 10 cases, and in 25 cases of NH through OPD follow-up, clinical improvement was noted in 20 cases.
