This is a case report of phenobarbital idiosyncrasy in a 2 months old female. The patient was admitted to our pediatric ward with symptoms of cyanosis, tachycardia and cheyne stoke respirations a few minutes after taking a small dose of phenobarbital. A case of infantile cortical hyperostosis is reported. The patient was 2 months old female who complained of mild fever and irritability, especially when changed her diaper. X-Ray showed marked periosteal elevation on the mandible, upper and lower extremities. Other laboratory tests were almost within normal limit. A brief review of literatures was made.
